For those who have more than one vendor number requiring the completion of the remediation process, one remediation form (and supporting documents) is required per vendor number. Attached are copies of the residential and non-residential remediation forms, as well as a form further explaining each of the 10 federal requirements (1 through 5 apply to all, and 6-10 only apply to residential providers). Below is a list of some acceptable types of supporting documents:
· Segment from program design that includes language supporting compliance.
· Provider policy or procedure that includes language supporting compliance.
· Segment from client handbook that includes language supporting compliance.
· Schedule or calendar of activities supporting compliance.
· Monthly client meeting record. (Names of persons served removed)
· Staff training curriculum and schedule.
